How does someone qualify for nursing home Medicaid in Louisiana?
A Louisiana resident qualifies by meeting nursing facility level of care, intending to stay at least 30 consecutive days, and meeting the financial rules: in 2026 countable income up to $2,982 a month (three times the SSI federal benefit rate), or higher income with enough medical expenses to spend the excess down, and countable resources up to $2,000 for an individual or $3,000 for a couple in the same facility.
Louisiana's spend-down route matters: a resident whose income is above the limit is not shut out if the cost of nursing home care brings income down to the eligibility level. Countable income includes Social Security, pensions and retirement benefits, veterans' benefits and other unearned income.
What happens to the spouse who stays home?
Federal spousal impoverishment rules protect a share of the couple's assets and income for the spouse at home: in 2026 the community spouse resource standard runs from $32,532 to $162,660, and the minimum monthly maintenance needs allowance is $2,705 from July 1, 2026, up to a maximum of $4,066.50.

| Community spouse resource standard, maximum | $162,660 |
| Minimum monthly maintenance needs allowance | $2,705.00 (effective July 1, 2026) |

How those standards apply to a particular couple depends on the snapshot of assets on the first day of continuous institutionalization and on Louisiana Medicaid's calculation. An elder law attorney can explain the options before the application; a mistake here can be expensive.
What is the look-back period for Louisiana Medicaid?
Louisiana Medicaid reviews transfers of assets for less than fair value made in the 60 months before the application, and a gift or below-market sale in that window can cause a penalty period during which Medicaid will not pay for nursing home care.
Applications ask for bank and financial statements going back that far, and missing documents are a common reason applications stall. Start collecting statements early, and keep records of any money spent on a parent's care.
How much does a Louisiana nursing home resident on Medicaid keep each month?
$45 a month: Louisiana Medicaid raised the Personal Care Needs Allowance for long-term care residents from $38 to $45 on July 1, 2025, and raised the state-funded Optional State Supplement for eligible residents from $8 to $15 the same day.
The allowance covers personal items the facility fee does not, such as clothing, a haircut or a phone. Most of the resident's remaining income goes toward the cost of care. Ask the nursing home how it holds and accounts for residents' personal funds.
Is the home protected when a parent goes into a nursing home?
Often, at least for eligibility: the home is generally excluded when the applicant intends to return, or a spouse or certain relatives live there, subject in 2026 to a $752,000 home equity interest limit.
Eligibility and estate recovery are different questions. A house that does not count for eligibility can still be subject to Medicaid's claims after death in some circumstances. Ask an elder law attorney about your family's situation before transferring or selling a home.
How do we apply for nursing home Medicaid in New Orleans?
Apply through the Medicaid Self-Service Portal, by phone at Medicaid Customer Service 1-888-342-6207, by mail or fax using LDH's application, or in person at a local Medicaid office; Medicaid also sends a “Ways to Apply” letter when a person is admitted to a nursing home.
Answer every question and send the proof requested; Medicaid cannot decide eligibility without it. Denials can be appealed to the Division of Administrative Law.
